Insurance and Billing

We participate with several managed care plans including HMO, PPO and POS as well as Medicare, Blue Cross / Blue Shield, and Florida Work Comp.

Please bring your insurance card with you to each and every appointment. It is imperative that we confirm your continued coverage, and if you do not bring a card with this information, and have not made prior arrangements, you may be responsible for the full charge of your visit.

We also ask that you be prepared to pay any co-pay or amount due at the time of service. You may pay by cash, check or credit card.

We recognize that insurance and billing have become extraordinarily complex and we will be glad to assist you with any difficulties you may encounter. You may contact our billing department at extension 252.

Please note: We assess a fee of $45 for the first missed visit and a $60 fee for all subsequent missed visits, unless your appointment has been cancelled at least 24 hours prior to your scheduled time. Our patient's time is very important to us and we feel that arriving on time and being able to predict who will arrive is an integral part of the physician staying on schedule. In addition, it is frustrating to both patient and doctor when we have no appointments available, yet the physician is expecting a patient who then does not show up for his or her appointment. We appreciate your understanding.
